MVP Survey Example
Marisa recommended Jotform.com to create your MVP Survey. Been using it for nearly 10 years. Great tool and there is a FREE version (see below).

Interactive Checklist
One of the things that Jotform does really well is let you select what question or information someone sees based on their answer to a question. I find it is an effective way to help people recognize that they have a problem as well as provide some tips and a call to action that might generate a connection. NOTE: This checklist was created about 8 years ago so info is a bit out-of-date.
